COMMON AGENDA

GOAL: By 2020, all children with asthma lead full, active lives with minimal symptoms. 25% reduction in ED visits, hospitalizations and school absences.

PROCESS OVERVIEW

Established in November 2016. the Staten Island Asthma Coalition is currently lead by the Stat-en Island Borough President’s Office, Staten Island Performing Provider System and the Rich-mond University Medical Center.

Focus: 4 Key Strategies and Workgroups: 
  1. Care Coordination & Home Environmental Intervention: Linking families to care coordination, asthma home visit program and remediation services. 

  2.  Clinical: Ensuring implementation of asthma guidelines by creating asthma patient registries, and tracking quality measures through electronic health records (EHR). 

  3.  Schools & Community: Strengthening communication and ensuring linkages to primary care and community services for all children with asthma.

  4.  Data: Evaluating asthma utilization trends to inform the strategies of our subcommittees. 
​The coalition is dedicated to improving the health of all children with asthma on Staten Island using the collective impact approach with cross sector collaboration.
KNOW THE FACTS!
  • Over 200,000 children suffer from asthma citywide.

  •  Approximately 4,000 school aged children on Staten Island have asthma. Approx. 1,200 of those have persistent asthma, with symptoms occurring at least twice a week.
     
  •  Asthma is one of the most common health issues in children in the United States. It’s the number one reason children are hospitalized.

  •  In 2014, nearly 100 children ages 5-14 years old were hospitalized and over 700 emergency room visits occurred due to asthma on Staten Island.
